Pelvic organ prolapse occurs when one or more of the pelvic organs, such as the bladder, uterus, or bowel, move downwards due to reduced support from the pelvic floor and surrounding tissues. This can lead to a feeling of heaviness or bulging in the vagina and may affect bladder, bowel, or pelvic comfort.
Pelvic organ prolapse can develop gradually and is commonly associated with pregnancy and childbirth, hormonal changes, connective tissue factors, and previous pelvic or abdominal surgery.
Pelvic organ prolapse may involve different organs and can vary in severity.
Where the bladder presses into the front wall of the vagina.
Where the uterus descends into the vaginal canal.
Where the bowel presses into the back wall of the vagina.

Symptoms can vary depending on the type and degree of prolapse and may fluctuate throughout the day. You may experience one or more of the following:
A feeling of heaviness, dragging, or pressure in the pelvis
A sensation of a bulge or something coming down in the vagina
Symptoms that worsen with standing, lifting, or at the end of the day
Bladder symptoms such as leaking urine, urgency, or difficulty fully emptying
Bowel symptoms such as difficulty opening the bowels or needing to press the back wall of vagina to empty
Discomfort or reduced sensation during intercourse
Lower back or pelvic discomfort

Assessment of pelvic floor muscle strength, coordination, and control

Mobilisation of joints where movement restrictions contribute to symptoms
Assessment and treatment of related musculoskeletal issues, such as back or hip pain
Guidance on posture, breathing, and pressure management
Whole-body rehabilitation and strengthening
Movement-based exercise, which may include Pilates-based rehabilitation
Support with lifestyle factors such as activity modification, load management, stress, and sleep
